This letter of certification is in effect from 06/26/2007
to
06/26/2010.
One month prior to the expiration date of this letter of certification, your
company is required to submit a Recertification Affidavit of Continued
Eligibility form for review as to your status to remain in the EDGE
program.
However, you must notify this division immediately if any changes occur within
your company that effect ownership, managerial and/or operational control within
(30) days of such changes. In addition, notification should be provided for
changes to the company's name, business address, telephone number(s), principal
products/service, etc. Failure to notify this office of changes or to provide
the Recertification Affidavit of Continued Eligibility form in a timely manner
can result in revocation of your certification status.
